Scientific Overview
SS-31, also known as elamipretide, MTP-131, or Bendavia, is a mitochondria-targeted tetrapeptide. It is designed to localize to the inner mitochondrial membrane, where it interacts with cardiolipin, a specialized phospholipid that plays an important role in mitochondrial structure and energy production.
Cardiolipin is essential for the organization and function of the electron transport chain, the mitochondrial system responsible for producing ATP. When cardiolipin is damaged or disrupted, mitochondrial energy production can become less efficient, oxidative stress can increase, and cellular function can decline.
SS-31 is studied because it binds to cardiolipin and may help stabilize the inner mitochondrial membrane. Mitochondrial dysfunction is connected to fatigue, reduced stamina, impaired recovery, increased oxidative stress, muscle weakness, aging-related decline, and disease processes involving energy failure.

Evidence Strength
Preclinical mechanistic evidence is strong: SS-31 reliably binds cardiolipin, supports mitochondrial membrane potential, reduces mitochondrial ROS, and improves bioenergetics across multiple models.
Human clinical relevance is strongest in Barth syndrome, a rare mitochondrial disease. In September 2025, the FDA granted accelerated approval to Forzinity (elamipretide injection) as the first treatment for Barth syndrome, based on improved knee extensor muscle strength. This approval does NOT extend to longevity, bodybuilding, wellness, recovery, or general mitochondrial optimization.
